Referrals and Electing Hospice Care

From physicians to friends, family members or neighbors, anyone can make a referral for hospice care. Valor HospiceCare & The Institute for Palliative Medicine team members are available by phone 24 hours a day, seven days a week. When a referral is made, a Valor HospiceCare representative will schedule a visit with the patient and/or loved ones. Care usually begins within the day of a referral.

There are three steps to receiving hospice care:

Step 1) Place a call to Valor HospiceCare & The Valor Institute for Palliative Medicine
Step 2) Our Intake Coordinator will contact all responsible parties to coordinate a family consultation and/or a patient assessment
Step 3) Our hospice team consults, evaluates and initiates care, if appropriate

If your doctor believes that further treatment is unlikely to alter the disease process, hospice care can offer the alternative of comfort care in your home.

General Guidelines: When is Hospice Appropriate?

· The person has a life limiting prognosis with six months or less to live
· The patient requires frequent visits to the Physician's office or emergency room
· The patient is becoming weaker and requires more assistance with activities of daily living
· A significant decline in the patient's physical or mental health state
· The patient's disease is progressing at a rapid rate or new diagnosis are added
· The patient has lost unintentional weight
· The patient's level of mobility has declined and requires more assistance with daily living
· The patient has recently chosen to stop aggressive treatment options

Paying for Valor HospiceCare Services

Hospice is an 100% covered benefit of Medicare for services related to the terminal illness or disease and pre-authorized by the Hospice team. Valor is an AHCCCS/ALTCS (Arizona's Medicaid Program for Long Term Care) provider and accomodates most private insurance companies. We also offer additional payment options, including services for managed care (HMO, PPO, etc.) and private pay. In cases where no insurance is available, our services can be provided regardless of the patients ability to pay.

More than 95% of Valor's clinical care is provided in the home setting of a patient. If patients need short-term acute or chronic symptom care beyond routine care in the home setting, Valor HospiceCare has arrangements with local Medicare-certified Skilled Nursing Facilities. Qualified, authorized facilities on our program are hand selected by our standards for their quality of care reputation and geographic convenience to our patients, caregivers and families. Valor is currently building inpatient centers to provide additional care in our communities.

When medically necessary, Medicare, Medicaid/AHCCCS and most plans cover inpatient or respite care. If a patient stays in an authorized Skilled Nursing Facility, or Valor Inpatient Center, beyond medical need, the cost of room and board become the responsibility of the patient and family.
