Clinical
& Social Services Positions
NEW! PatientCare Services Director (RN) - Tucson Office
Leadership position available for responsibilities of directing and supervising the clinical operations of the team, in accordance with industry standards, regulatory agencies, policies and procedures and Valor's "Philosophy of Care". This role requires a current, unrestricted Registered Nurse (RN) license in the state of Arizona. Bachelor degree, Master's preferred and clinical experience is essential. Must have at least three years of hospice management experience, including a background in clinical excellence and business/operations management. Excellent communication skills will be necessary. Computer proficiency is a must. Primary accountabilities include:
· As a company expert on Medicare conditions of participation, and in collaboration with Medical Directors and Administrator, proactively coordinates standards of care and new programs that uniquely positions company in communities served
· Plan, develop, organize, direct and evaluate assigned clinical functions and performance of team
· Drive and support census development through "open access" initiatives of PatientCare, Marketing & Admissions Programs
· Proactively educate, assist and promote professional growth for all clinical staff
· Supports development of palliative care programs, with company medical directors and nurse practitioners, and other IDG team members, for care in private homes, skilled nursing facilities, assisted living facilities, adult care homes and other long term care facilities
· Oversee business components of programs and ensure control and evaluation of patient cost factors
· Collaborate with CQI & Education Coordinator in Continuous Quality Improvement measures
PatientCare Coordinator (RN)
Responsible, as a member of the interdisciplinary group and Governing Body, for the delivery, coordination and supervision of hospice services delivered by respective team consistent with Valor HospiceCare policies and procedures. Responsible for the management, planning, implementation, delivery and supervision of direct clinical services provided to patients and families, the coordination of the Valor HospiceCare PatientCare program, and the effective and efficient use of resources.
· Conducts interdisciplinary group (IDG) meetings for team patients
· Manage RN Case Managers and LPNs for clinical management of plan of care
· Oversee Social Services staff and programs
· Direct Certified Nurse Assistant visits in accordance with RN Case Manager oversight and regulations
· Ensure Therapist duties are utilized per plan of care
· Collaborates with Admissions Nurses and marketing for effective admissions and initial plans of care for patients and families
· Utilizes labor resources for various care levels and ensures that recertifications are established for qualified patients
NEW! Nurse Case Manager (RN) - Sierra Vista Office
Responsible as a member of the interdisciplinary group to the PatientCare Coordinator and the attending physician for the provision of skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families consistent with Valor HospiceCare & PalliativeCare policies and procedures. As Nurse Case Manager, responsible for identifying patient/family needs and for providing supportive care and appropriate services in accordance with the attending physician's orders and the interdisciplinary group plan of care. Must be computer literate. Primary responsibilities include:
· Educating patients, families and caregivers
· Providing a care plan and implementing the care
· Performing on-site evaluations of CNAs and volunteers
· Providing necessary information to the PatientCare Coordinator
· Supporting on-call and admissions processes
· Monitoring the re-certification process
· Participating in CQI assessments and routine chart reviews
· Assisting with the orientation of new team members
· Bilingual Spanish a plus
C.N.A. Certified Nurse Assistant
Under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to the patient.
· Provides emotional support and compassionate care to the patient and family unit
· Maintains required competency and skills.
· Reports any changes in the patient’s condition or family situation immediately by calling the Hospice office and notifying the PatientCare Coordinator, Team Leader and/or the Nurse Case Manager.
· Practices Universal Precautions according to OSHA regulations and HCI policy
· Adheres to all CHCI policies and procedural guidelines.
· Attends at least twelve (12) hours of continuing education programs or approved audiovisuals per calendar year.
· Maintains current licensure as required by the State Board of Nursing.
· Maintains current knowledge and practice of infectious disease protocols
NEW! On-Call Nurse – Sierra Vista Officeice
Responsible as a member of the interdisciplinary team to the PatientCare Coordinator for the provision of skilled nursing assessment and hospice care in order to maximize the comfort and health of patients and families consistent with Valor HospiceCare policies and procedures. As On Call Nurse, responsible for providing skilled nursing, intervention, supportive care and admissions in patients’ homes for weekend, holiday, evening and night on-call duty in accordance with the Hospice Interdisciplinary plan of care. Primary responsibilities include:
· Assuming responsibility for evening, weekend and holiday on-call duty as scheduled
· Assessing, planning, implementing and evaluating phases of the nursing process, in collaboration with assigned RN Case Managers
· Initiating communication with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family
· Obtaining data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate into the plan of care
· Maintaining up-to-date patient records
· Monitoring physical care
· Responding and providing direct care as appropriate
· Providing appropriate counsel
· Supporting admissions team
Medical Social Worker (LCSW/LMSW)
The Medical Social Worker is responsible for the implementation of standards of care for medical social work services. A degree in Social Work from an accredited educational institution is necessary, and a LCSW/LMSW is preferred. A demonstrated ability in casework, counseling and group work, including one year of clinical experience in hospice or health care setting.
· Maintains the standards of Social Work excellence in accordance with the principles set forth by the Arizona State Health Department, and agency policies and procedures.
· Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of goal-directed family care plan.
· Observes, evaluates and brings to team conferences information regarding psychosocial, spiritual, physical and financial conditions affecting the patient and family.
· In addition to participating in marketing referral and admissions programs, assists with community outreach and census goals.
Crisis
Intervention Team (CIT) Coordinator (RN)
Coordinates continuous care program for locations out of Tucson Central
Office for crisis intervention for pain management and symptom control.
Works with Nurse Case Manager to ensure continuity of care whether in
a private home or nursing home or assisted living facility. Recruits
and develops CIT team members for 24-hour nursing and aide care. Strong
planning and organizational skills required, including extensive use
of computer programs of MS Word and Excel.
Crisis
Intervention Team (CIT) Nurses (LPN)
Highly flexible schedules available for patient services for the Continuous
Care level, per Medicare. Assists with continuous care in a 24-hour
period. Works with Nurse Case Manager to ensure continuity of care for
this pain and symptom management crisis. Great LPN position for semi-retired
nurses who want to call their own schedules with the CIT nursing pool.

Associate Medical Director
Our growing organization is looking for Medical Directors/Associate Medical Directors/Hospice
Physicians. Position requires experience in hospice, palliative medicine
or geriatric care programs. Will support managing location interdisciplinary
group (IDG) meetings, as well as directing patient care for patients
in private homes, skilled nursing facilities, adult care homes and assisted
living facilities. Will also assist with company's palliative care programs,
including in-home patient evaluations and current patient consultation
visits. Through community outreach program, will assist with education
of Medicare hospice benefit with areas physicians and other healthcare
professionals. Must be well organized, able to clearly articulate the
hospice benefit and have experience in supporting top level strategic
plans. Position requires Physician to carry own malpractice/liability insurance for non-hospice duties. Board certification in Internal Medicine and/or Hospice and Palliative
Care is preferred.
Nurse
Practitioners - Hospice and Palliative Care Programs

We are seeking NPs to become part of our unique, growing organization
working with the medical director and acting as a resource for staff
members. Will also support direct care programs with managed care (HMO,
PPO) partners. Bachelor's and Master's degree required along with Advanced
Registered Nurse Practitioner license. 3-5 years related experience
with hospice, skilled nursing or assisted living care and home health
experience. Excellent communication, team work and problem solving skills
required. Responsible as a member of the interdisciplinary group and
attending physician for management of chronic illness, patient education
and health maintenance in private homes, skilled nursing facilities,
adult care homes and assisted living facilities, consistent with Valor
HospiceCare & PalliativeCare policies and procedures. Will diagnose
illness and prescribe medication, therapy and other diagnostic testing.

NEW! Information Systems Coordinator - Tucson Office
Operates as a member of the administrative and operations support staff under the direction of the Controller and in collaboration with the Executive Director. Installs, modifies, and makes minor repairs to computer hardware and software systems and provides technical assistance and training to internal and external users. Ensure data integrity for company ERP system.
· Leads all computer system setup, maintenance and installation, including computer user profiles, access rights and directories security in collaboration with human resources and operations for all Valor locations.
· Responsible for overseeing the network computational and communications systems and all related training.
· Responsible for ensuring HIPAA regulations are applied.
· Will participate in installation of new Enterprise Resource Planning (ERP) software for capturing all daily census, patient data, additional data required and other plan of care components, as well as integrating with Valor’s computer finance programs, such as Peachtree or MAS90.
· Will participate in Valor Institute – New Employee Orientation (NEO) program for Computer 101 and other classes, including training for Word, Excel and other Microsoft programs.
· Will oversee all network integrity, with human resources, for computer systems, including password maintenance database/book.
