Southwest Hospice Foundation      

Our mission is to enhance communities through our provisions for all residents to access exceptional hospice care through the end-of-life.

To support the community, currently in development, the non-profit Valor Hospice Foundation provides support for the extraordinary work of Valor HospiceCare & PalliativeCare staff and volunteers.

The Foundation, a 501(c)(3) organization managed by community leaders, allows tax deductible contributions to develop and maintain philanthropic causes. Working with individuals, businesses, corporations and foundations, we offer:

   · conferences, seminars and community education that focus on quality of living      and encompass practical, spiritual and emotional concerns.
   · the support of indigent patients on Valor HospiceCare & PalliativeCare's      program.
   · grants and educational assistance to qualified students who have an interest in      hospice and palliative care in our communities, and are in the process of      obtaining degrees and/or certifications.

Memorial Services and The Jocelyn Price Memorial Fund

As part of our Bereavement Program and as a service to the community, Valor HospiceCare & PalliativeCare's Memorial Service will honor the patients that we serve. If you would like to send contributions for The Jocelyn Price Memorial fund, please mail to Valor HospiceCare & PalliativeCare, 1760 E. River Road, Suite 230, Tucson, Arizona 85718, Attn: Administrator. Please make sure that you include the name of person in memorial and your contact information.

Memorial gifts or donations to Valor HospiceCare & PalliativeCare or Southwest Hospice Foundation are currently not tax deductible. The State of Arizona recognizes the Foundation, however, the IRS is currently reviewing the application. Please contact your tax advisor for questions.
